APPOINTMENT INFO


Before your first appointment at DeVito and Alvarado Pediatric Associates, we encourage you to take a moment to review our office policies. You can also expedite the check-in process by printing out and completing the Initial Visit Medical Information Form, the Patient Registration Form and the Notice of Privacy Practices Form. If applicable, you may submit the Authorization for Release of Medical Records Form to your previous health care provider so that your child's medical records may be sent to our office. Our goal is to make your experience at our office as seamless as possible so your child can get in to see the doctor quickly!

  • Schedule an Appointment

    At DeVito and Alvarado Pediatric Associates, we strive to honor all scheduled appointment times.  We understand that circumstances will arise and request that you notify us of any last minute changes as soon as possible.  Our computerized scheduling system is designed to minimize waits and overbooking.  Patients who miss appointments without calling or cancelling may cause delays.  If you must miss an appointment, please call to cancel and reschedule at least 24 hours in advance.  Missed appointments delay needed vaccinations and keep us from providing the best pediatric care.  We ask for your cooperation when making an appointment and request that you make note of the date and time.  We will call you the day prior to confirm your appointment as a reminder.

  • Appointment Required

    All visits require an appointment. We may be unable to see your child if you walk in without an appointment and may ask you to return later in the day when the schedule allows.

  • Sick Appointments

    We make every effort to schedule appointments for ill children on the day that you call us. Please call our office ahead of time so that we can minimize the time that you will spend waiting.


    If you have a medical emergency during office hours, we will do our best to see your child immediately or refer you to the appropriate facility.

  • Well Appointments

    Did you know regular well child visits are one of the most important things you can do to keep your child healthy? Well child visits also provide you with the opportunity to discuss any questions or concerns you may have about your child’s health.


    Your child should have a well child visit at the intervals listed below. We recommend that you call our office 3 months in advance to schedule an appointment with your child's primary care provider on the date that best fits your needs.

  • Emergencies

    Should a medical emergency arise, please call DeVito and Alvarado Pediatric Associates anytime- 24 hours a day, 7 days a week. If the office is closed, our answering service will take your call and one of our doctors will be paged and will call you back promptly. If the emergency is life threatening, please call 911 immediately.

  • Walk-Ins

    DeVito and Alvarado Pediatric Associates does not have designated walk-in hours.  We prefer that an appointment be made in advance so that we can minimize your wait time.  We understand that situations do arise. Therefore, patients who walk in with a non-emergency condition will be fit into the existing schedule as best we can.
