Billing Policies

DeVito and Alvarado Pediatric Associates, PLLC participates with most insurance companies. Please visit our Insurance Information section for a complete list.

If your policy requires a copayment, this must be made at the time of your child’s appointment.

If your plan has a deductible, coinsurance or any non-covered procedure, you will receive a bill and payment is expected within 30 days.

All insurances indicate a specific amount of allowable preventative care or well visits. Please contact your insurance company to determine how many well child visits are allowed and for what period of time.

It is your responsibility to notify us of any changes in your insurance to ensure that the claim is sent to the correct insurance plan so that you are not held responsible for the charges.

Most importantly, it is your responsibility to understand your insurance policy. Kindly check with your insurance company to determine the specific coverage benefits and limitations your plan provides.
